To help you thrive, we work with Community SWITCH (Supporting Wellbeing Improving the Community’s Health). This service connects you to local support and activities that can improve your overall quality of life.

This complements your existing healthcare by focusing on a holistic, person-centred “What matters to you?” approach, rather than just “What is the matter with you?”

How can we help?

If you use this service, a member of our Social Prescribers can work with you to:

  • Find local activities: Connect with community groups, exercise classes, or wellness programmes.
  • Set personal goals: Create a plan to help you feel more in control of your health.
  • Access practical support: Find help with things like housing, debt, or loneliness.
  • Stay on track: Have regular check-ins to see how you are getting on and adjust your plan if needed.

How can I help myself?

You do not need to see a GP to get help from Community SWITCH. You can refer yourself or use their online tools to find resources that fit your needs.
