  • West Parley and parts of Parley Cross (generally north of the River Stour)
  • Tricketts Cross

Steps to registering with us

1. Check your postcode on our Catchment Map

Before starting your application, please check that your address is within our boundary.

2. How to register

You can register with us in two ways:

  • Online (Recommended): Use our secure digital service to register from home. Register with us online here>>
  • In Person: Visit our reception desk to collect a paper registration pack.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

3. Completing your registration

Once you have submitted your form, please visit the surgery with:

  • Photographic identification (such as a passport or driving licence).
  • Proof of address (such as a utility bill).

Note: If you have difficulty providing these documents, please let us know. We will work with you to ensure you can still register.

Our Commitment to You

Equality and Respect: We do not discriminate on the grounds of race, gender, age, religion, sexual orientation, disability, or medical condition. Every patient is treated with equal care and dignity.

Choosing a GP: You are registered with the practice rather than a specific doctor. This means you can see any of our GPs. You may request a “preferred” doctor, but please be aware you may have to wait longer for an appointment with them.

Urgent Care (Triage): We offer a daily telephone consultation service with the Duty Doctor. A receptionist will take your details, and a doctor will call you back to discuss your needs. If the doctor needs to see you in person, they will give you an appointment time for that same day.
