We are confident these improvements provide a clearer, more convenient way for you to manage your health online.
SystmConnect has replaced E-Consult. Please click the link above to complete a short form for:
– Medication requests (48hr processing time)
– Fit note requests (can be back-dated, but not future-dated even over a weekend/bank holiday)
– or routine queries
Once submitted, it will be reviewed by the practice.
– Medication requests will be actioned, and we will only contact you if there is an issue. It will otherwise be sent to your nominated pharmacy.
– You will be contacted once your fit note has been actioned.
– For routine queries, you may be signposted to alternative services, receive a message from reception, or be offered an appointment.
You do not need to contact the surgery in any instance unless your symptoms change or worsen.
