Summary Care Records (SCR)
Your Summary Care Record is a digital version of your key health information (such as your medicines and allergies). It can be seen by authorised staff (such as A&E doctors or paramedics) who are treating you outside the surgery.
You have a choice. If you are happy for your information to be used in this way you do not have to do anything. If you have any concerns or wish to prevent this from happening, please complete a Data Opt-Out form. This stops your data from being shared outside of this GP surgery for anything other than your individual care.
National Data Opt-Out (Online)
The NHS uses some patient data to research new treatments and plan better services. You have the choice to stop your confidential patient information being used for purposes other than your own individual care.
